I've posted a photoshop mockup (based on the healthreform.gov site) for the UNINSURED LA webpage on kickstarter.com, where I am gathering starter funds for the Uninsured LA campaign:

UNINSURED LA is a solidarity movement for the uninsured and underinsured in support of California and national health care reform started by artists and community organizers in Venice and Santa Monica, CA.
