Health care reform needs a true single-payer system. This is the only way we will reduce health care costs and achieve universal coverage. Medicare is currently not single-payer in that you also need to pay for a private insurance supplement and then you have to pay for items not covered by Medicare or the supplement. We need to make one payment to Medicare and they take care of all the health care bills. Huge efficiencies can be achieved through computerization, standardization of records, smart cards, full access to generics, etc. Insurance companies make money by charging more and denying claims. We cannot trust the insurance industry to cut costs without a single-payer Medicare plan making all the payments.
The same is true of a public health care option which is similar to Medicare or what Congress gets except that the public option should not have to be subsidized by taxpayers.
